BHRT for Women
Treating the long perimenopause window before menopause.
Perimenopause treatment is for the years when cycles are changing and symptoms have already started. You do not have to be officially 'done' to be seen. The plan may include hormones, and it may start with sleep, thyroid, iron, and a better description of the bleeding.
Cycles get closer together, then skip, then surprise you. Sleep frays. Anxiety shows up in women who did not have it. Migraines can change. Some months feel premenstrual for two weeks. If that is you, you are not imagining a trend.
Fertility is not zero. Contraception still matters if pregnancy would be a problem. Mention it. A hormone plan and a birth-control plan can overlap, and they can also conflict.
FSH and estradiol bounce in perimenopause. A normal draw on a good week does not erase six bad months. The pattern of periods and symptoms carries more weight than a single afternoon lab, which is why the history is long on purpose.
Thyroid disease and iron deficiency impersonate this phase. Both are worth checking before anyone adds estrogen and calls the fatigue hormonal.
Sometimes a clinician uses a hormonal approach to flatten the swings. Sometimes the first move is progesterone at night for sleep, or treating vaginal symptoms locally, or leaving hormones alone and fixing sleep apnea. The point of a 45-minute visit is to pick among those, not to upsell a stack.
Track two months of bleeding before you come if you can. Dates and flow. It changes the recommendation more than a list of adjectives.
Perimenopause is the phase where women get caught off guard, because fertility is reduced but not zero, and a 'skipped period' can be a pregnancy rather than the transition. If a pregnancy would be a problem, contraception still matters, and it needs to be part of the plan rather than an afterthought. Saying so lets the clinician choose an approach that covers both the symptoms and the birth control at once.
That overlap can be an advantage: a hormonal method sometimes does double duty, flattening the swings while also preventing pregnancy. It can also conflict, which is exactly why you should bring the name of anything you are already taking and not stop it the week before the visit 'to see what your hormones do.' Let the clinician see the real baseline.
Treatment is provided through Jonathan Larson MDPC. Prescriptions require a clinical evaluation; medication is dispensed only when deemed medically necessary. Information here is educational and not a substitute for medical advice.
