Energy & Vitality
Workups and treatment plans for chronic fatigue.
Chronic fatigue treatment starts by separating 'I am busy' from a medical fatigue: exertion that wipes you out, sleep that does not restore, feverish days, or a decline that has a date. The workup is labs and history. A stimulant is not the front door.
Sleepiness wants a sleep history and sometimes a study. Muscle weakness wants a different exam. Brain fog on a GLP-1 wants a look at how little you are eating. Low mood wants to be named as mood. Post-viral fatigue wants pacing, not a pep talk.
Bring a timeline. 'Since October, after the illness' is usable. 'Forever' is harder, and still worth unpacking into years.
CBC, ferritin, TSH, B12, a metabolic panel, and sometimes vitamin D or celiac testing. Not fifty tubes because a wellness site sold a bundle. Add tests when the history points.
Normal labs do not mean you are fine. They mean the common deficiencies are not the answer, and the next step is sleep, mood, medication review, or a local referral. That is progress.
Treat what you find. Pace what you cannot treat yet. Caffeine strategies and a fantasy morning routine will insult someone who is canceling work. The visit should sound like your week.
If you came for a NAD infusion, you can ask, after the workup. Leading with the infusion skips the part that might fix you.
For some fatigue, especially the post-viral kind, the standard advice to just exercise more is actively harmful, because exertion is followed by a crash that costs days. Naming that pattern matters, because the plan for someone who crashes is the opposite of the plan for someone who is simply deconditioned and under-slept. One needs a careful ceiling; the other needs gentle movement.
Pacing is unglamorous and it feels like giving up to people used to willpower solving things. It is not. It is spending a limited budget of energy on purpose instead of blowing it and paying for a week. A visit that respects that will sound like your actual life, not a motivational poster.
Treatment is provided through Jonathan Larson MDPC. Prescriptions require a clinical evaluation; medication is dispensed only when deemed medically necessary. Information here is educational and not a substitute for medical advice.
