Semaglutide & Insurance

This is the confusion. A plan may cover semaglutide when it is prescribed for diabetes and not when it is prescribed for weight, or it may require a prior authorization that reads like a second job. Your clinician documents what is true. Nobody should rewrite your chart to fit a billing code.

If you do have diabetes, say so, and bring the last A1C. If you do not, do not suggest that someone code it anyway. That request will be refused.

Appeals take time. Do not pause a medicine you are already stable on, from another prescriber, without asking that prescriber. Switching clinics in the middle of an appeal can make the paperwork worse.

There is a temptation, when a plan covers a diabetes indication but not a weight one, to ask someone to code it the convenient way. That request will be refused, and for your protection: a chart that does not match the truth is insurance fraud exposure for you as much as for the clinic, and it can unravel later coverage when the records are reviewed. The clinician documents what is actually true, full stop.

The better path is to work the real levers. If you genuinely have diabetes, bring the A1C; if a prior authorization is the obstacle, the fix is chart notes that meet the plan's rules, not a rewritten diagnosis. When a plan simply excludes weight-loss drugs as a category, the honest move is a cash conversation, not a coding game that puts you at risk.
